I acknowledge that I have sufficient liability insurance of the boat I will use in any Championship Teams(CT) tournaments. I understand and agree to grant TMC the unconditional right to use my name, picture or any comments without receiving any compensation. I furthermore understand one team member designated by the Tournament Director may be subject to take a polygraph examination. Failure to take or pass the polygraph will result in disqualification and the possibility of not fishing in any TMC events in the future. I further agree to abide by the CT Rules and Regulations. is to certify that I, as a parent/guardian with legal responsibility for
this participant, do consent and agree to his/her release as provided on
opposite side, and, for myself, my heirs, assigns, and next of kin, I
release and agree to indemnify and hold harmless Tournament Marine,
Corp. (TMC) from any and all liabilities incident to my minor child's
involvement or participation in these programs as provided above, even
if arising from the negligence of TMC, to the fullest extent permitted
